Care & Support Practitioner
Location: Hestia House
Salary: £27,111 per annum
Hours: 37.5 hours per week (10am – 6pm, may be required to cover occasional weekends)
Job Type: Permanent/Fixed Term

As a Care & Support Practitioner at YMCA Together, you will be an empathetic and caring individual committed to providing unconditional support to those experiencing mental health issues. Your role will involve creating a safe environment that empowers residents to make positive changes in their lives.
What your week may look like
- Introduce and build relationships with new residents, including inducting them into the service.
- Ensure residents understand their rights and responsibilities, including engagement with care assessments and statutory services.
- Support residents in maintaining independence and achieving their goals as outlined in their care plans.
- Update and communicate resident risk assessments, including positive risk-taking and medication management, in line with YMCA policies.
- Refer to care and support plans for individual needs.
- Provide personal care as detailed in care plans, ensuring dignity and respect.
- Perform delegated health tasks as required, with training provided.
- Communicate with partner organizations and handle out-of-hours statutory and non-statutory agency contacts when needed.
- Maintain up-to-date records on paperwork, support planning systems like DAVE, and resident files.
- Participate in cleaning duties and support residents with room management.
- Assist in planning and delivering evening group activities.
- Provide information and advice according to care plans.
- Motivate residents to achieve their goals and involve them in service planning through meetings.
- Handle complaints and safeguarding concerns promptly.
- Share accurate and quality information about residents with line managers as per procedures.
- Demonstrate creativity and commitment in supporting vulnerable individuals.

The skills we are looking for
- Qualification or experience in Care or Support Services
- Experience in providing personal care to vulnerable adults
- Knowledge of Health & Safety issues
- Understanding of complex needs such as mental health, homelessness, substance misuse, and offending
- Knowledge of Adult safeguarding protocols
- Ability to handle challenging behaviors with dignity
- Skill in conducting risk assessments
- Effective care and support planning skills
- Excellent IT, communication, and interpersonal skills
- Strong administration and record-keeping abilities
- Ability to plan and deliver workshops and sessions
- Creativity and problem-solving skills
- Ability to work independently and prioritize tasks
- Commitment to improving lives and personal development
- Energetic, motivated, and respectful of YMCA values
Nice to have
- Qualifications in Social Care, Health, or Mental Health
- Experience in substance misuse, welfare rights, or related fields
- IT qualifications like CLAIT, ECDL, ITQ
- Experience in a Care or Support setting, especially in CQC-registered environments
- Experience with end-of-life care, resettlement, or supporting through the Care Act
