An exciting opportunity has arisen within our well established Integrated Childrens Community Nursing team to introduce a new post into the Complex Care element of the team. The post holder will support the Advanced Clinical Practitioners (ACP) to develop and deliver an innovative specialist service for children, young people (C/YP) and their families who require specialist care.
This will be a challenging role that includes offering sensitive support, advice to families & C/YP who have Complex Health Needs supporting the (ACP) and the Childrens Community Nursing Team (CCNT) in supporting the clinical assessment and prescribing for acute episodes and ongoing after care.
The post holder will act as a point of contact for families who remain on ACP’s/CCNT’s caseload. They will provide specialist information, advisory and training resource to schools and families and other voluntary and statutory service providers.
They will develop links between disciplines and agencies to ensure a seamless services for clients. The post holder will continually look to develop flexible, innovative ways of working with families, statutory and voluntary bodies always ensuring child and their family centred service development.

- Autonomous practitioner responsible for planning and organising patient care, working within scope of own practice.
- Provide and support an ACP led service that will triage, clinically assess, diagnose, and treat children and young people with acute and complex care needs.
- Attend child protection, core group and family support meetings, participating in the decision-making process.
- Support children, young people and their families at home, in clinic and other settings and in conjunction with other professionals providing a link and liaison role that supports families to access appropriate services for their needs.
- To provide individual support to assist the patient and family to development knowledge in regard to specific needs and diagnosis and also around self-management skills.
- Provide specialist advice around key areas within the aspects of community and complex care.
- Practice Safe and effective Independent Non-Medical Prescribing– Assessing and Diagnosing any medical conditions that could specifically impact on the patients’ health and wellbeing.
- Adhere to the Medicines Management Code, Alder Hey’s NMP policy, NICE Guidelines and NMC Standards for the safe keeping and administration of medication. Ensure NMP prescribing is within their scope of practice.
- Hold a clinical caseload for lower acuity patients.
