MediSites360

The Opportunity:

Don’t just write code. Build a resilient, scalable backend platform, transform healthcare experiences, and shape the future of spatial computing. Big tech corporations can offer you a seat at a very large table, maintaining a tiny fraction of an existing codebase. We are offering you the chance to have a seat at the top table in a nimble, focused engineering team to architect and build a platform that directly impacts child healthcare around the world.

MediSites360 is pioneering the immersive digital front door for healthcare. We deploy

high-fidelity 3D spatial capture and interactive digital twins to build virtual hospital tours that reduce patient anxiety, solve complex wayfinding challenges, and transform patient care before families even step foot inside a hospital. Our flagship implementation at Alder Hey Children’s Hospital is already the largest virtual hospital tour of its kind in Europe, backed by POIZ (Paediatric Open Innovation Zone) Challenge Fund support and active engagement across multiple NHS Trusts.

To fuel our rapid commercial rollout and transition into a high-scale global platform, we are completely re-architecting our legacy backend and data infrastructure. That is where you come in.

The Role:

Working directly alongside our Technical Director, you will lead the core backend platform architecture, cloud infrastructure, data security, and API ecosystem for our next-generation product.

Architectural Role Division:

To support our technical evolution—including our pivot to a Gaussian Splatting (3DGS) tech stack—we have split our platform engineering into two tightly integrated specialist roles:

●     3D Spatial Web Developer: Focuses on client-side WebGL/WebGPU/Three.js 3D rendering engines, Gaussian Splat viewers, virtual staging UX, wayfinding overlays, and interactive spatial content.

●     Platform & Cloud Software Engineer: Focuses on core backend architecture, data pipelines for Gaussian Splat and Matterport asset ingestion, cloud hosting &

auto-scaling, data security, GDPR compliance, REST/GraphQL API design, and third-party integrations.

We believe in tight collaborative pairing rather than isolated handovers. While you handle backend compute pipelines, platform performance, authentication, and security, you will pair closely with the 3D Spatial Web Developer to ensure data pipelines and API boundaries feed seamlessly into front-end rendering engines.

Two-Stage Strategic Roadmap:

●     Track 1 (Immediate) - Modernisation & Standalone NHS Deployment: Refactor, stabilise, and rebuild our core Matterport-based backend functionality while systematically dismantling legacy PHP 7+/8+ and jQuery technical debt. Establish clean, robust API endpoints and deployment pipelines to support immediate NHS Trust rollouts and commercial deployments.

●     Track 2 - Full Migration to a Sovereign Gaussian Splatting Stack: Architect and execute a complete technical migration over the next 12 months to a fully proprietary, sovereign Gaussian Splatting (3DGS) platform. The primary imperative is building an in-house, end-to-end spatial engine that eliminates third-party dependencies and gives us total technical control over our 3D capture, processing, and serving pipelines (with potential multi-tenant SaaS expansion treated as a secondary future opportunity).

What Will You Be Doing:

●     Backend Architecture & Legacy Refactoring: Define, build, and maintain our core backend architecture (using Python/FastAPI/Django, Node.js/TypeScript, and PostgreSQL). Systematically migrate away from our legacy PHP technical debt into a clean, modular, microservices or serverless ecosystem.

●     Gaussian Splat & Spatial Ingestion Pipelines: Architect data pipelines to process, store, and serve large 3D spatial datasets, including Gaussian Splatting point clouds, Matterport API payloads, standard 3D meshes (.GLTF/.GLB), and raw multi-camera capture feeds.

●     Cloud Infrastructure & Scaling: Design high-availability, auto-scaling cloud hosting environments on AWS (ECS, Lambda, S3, RDS) using Docker containerisation, Kubernetes orchestration, and

●     Infrastructure as Code (Terraform).

●     Compliance, Data Security & GDPR: Implement bank-grade security protocols (Auth0 session validation, OAuth2, role-based access control, encrypted data storage) ensuring full compliance with NHS Information Governance, Data Protection Legislation, and UK GDPR standards.

●     Third-Party Systems & API Integration: Build and optimise seamless integrations with external services including Matterport APIs/SDKs, Cloudflare Stream (for

high-efficiency video distribution), AI text-to-3D pipelines, and clinical healthcare systems.

●     AI-Accelerated Engineering Velocity: Leverage modern AI tools (eg Cursor, Qodo or Windsurf) to maximise build velocity while maintaining rigorous testing, code review standards, and strict data safety boundaries.

Who You Are:

●     Hand-Coding & Fundamental Logic (Non-Negotiable): You are a programmer who truly understands how code works under the hood. You possess the deep syntax, memory, and algorithmic logic required to write clean, performant code by hand—and crucially, to critically evaluate, audit, and debug code written by others or generated by AI. You possess significant experience of hand-written coding syntax and logic, and can write clean, bug-free code from first principles, whether that is implementing custom data manipulation algorithms, or writing a bespoke script to patch a critical bug under pressure.

●     Modern AI Velocity with Human Oversight: You embrace modern development workflows (GitHub Copilot, Claude API, agentic coding tools) as productivity multipliers to ship fast—but you never treat AI as a black box. Your core software engineering fundamentals mean you remain the ultimate authority on code safety, architectural integrity, and security.

●     Educational Background & Technical Rigour: Graduate (Master’s preferred or strong BSc) in Computer Science, Software Engineering, or a related discipline with a rigorous grasp of software design patterns, object-oriented/functional paradigms, and distributed systems architecture.

●     Stack & API Proficiency: Proven experience building and maintaining production backend services and REST/GraphQL APIs—ideally using Python (FastAPI/Django) or TypeScript/Node.js—alongside relational databases (PostgreSQL), caching layers (Redis), and asynchronous data pipelines.

●     Legacy Untangling & Refactoring: Proven ability to dive into unfamiliar or legacy codebases (such as our existing PHP 7+/8+ backend), evaluate system behavior, and take a complex, messy problem or legacy debt apart, tracing execution paths line-by-line, isolating edge cases, and building elegant, long-term technical fixes.

●     Cloud Infrastructure & Security Awareness: Solid grasp of cloud deployment (AWS/GCP), containerisation (Docker), CI/CD pipelines, authentication mechanics (OAuth2, Auth0), OWASP security guidelines, and strict data governance (GDPR / NHS IG compliance).

●     Spatial Asset Pipeline Curiosity: Excited by the challenge of managing heavy spatial data payloads, camera capture workflows, and Gaussian Splat asset pipelines (no prior 3D graphics experience required, but comfort working alongside 3D developers is essential).

●     Security Mindset: Understanding of secure coding practices (OWASP Top 10), authentication protocols (OAuth, Auth0), audit logging, and handling sensitive data under GDPR / NHS governance rules, and help us achieve Cyber Essentials Plus in 2027.

●     Product-Minded & Confident Communicator: Possess the confidence to challenge convention, say when an approach needs to adapt for scalability or future-proofing, evaluate legacy systems objectively, identify new and exciting technologies that can advance our company mission, and communicate technical decisions clearly.

Why Join Us:

This is a career-accelerating showcase opportunity. By helping us rebuild our platform architecture from the ground up and transition to a global SaaS product, your career will scale directly with the growth of the company. As our technical team expands towards our 2029 SaaS roadmap, you will be ideally positioned to step into a Senior Engineering Lead or CTO-track role.

We offer a competitive startup salary, a supportive culture where innovation is encouraged, and above all, the opportunity to build technology that genuinely calms anxious children and families during critical healthcare visits.
