The difference you will make as an Activities and Dementia Coordinator

You will provide support, information and guidance to people with Dementia helping to maintain their independence, improving their sense of well-being and putting them in more control of their lives. This role will assist people with dementia to identify their needs and access to services. We’re looking for a caring, empathetic individual to ensure an activity plan is in place which is varied, exciting and dynamic to get the whole community buzzing. You will also be required to recruit, retain and support volunteers within the scheme to ensure a vibrant, active community. To be successful in this role you will genuinely care about people, be friendly and committed, and help our residents lead enjoyable and active lives.

You’ll actively encourage residents to get more out of life emotionally, physically and intellectually, and you’ll have a can-do attitude. The role is varied and challenging requiring you to provide a varied range of round the clock support and assistance. The post-holder with work with customers, family and friends to develop personalised care plans which go beyond meeting customers basic physical care needs and achieving their aspirations. The Dementia and Activities co-coordinator will ensure the care team deliver outstanding personalised care and support for residents; and ensure CQC, ARCO and relevant legislation and performance standards are exceeded.

About You

We are looking for someone with:

  • Track record of enthusiasm for dementia care and working with people
  • Knowledge and understanding of Dementia, and Dementia care and support
  • Knowledge and experience of CQC standards and CQC Compliance
  • Ability to drive a mini bus
  • Be a Dementia Friends Champion or willingness to attend this course

Role Profile

Create individual and group engagement and activity plans including: Life books, Life stories, Memory boxes, intergenerational and therapeutic activities related to individuals’ life, persona and aspirations

  • Maintain a caseload of customers who have dementia or are affected by dementia
  • Deliver the Dementia Friends awareness sessions throughout One Housing
  • Ensure Dementia assessments, care and risk planning meets best practice and customers’ aspirations
  • Act as a resource and support for the person with dementia, families/friends and staff
  • Research, fundraise, plan, coordinate and deliver a programme of activities
  • Maintain and manage appropriate systems to record and monitor 1-1 and group activities
  • Carry out risk assessments of activities, events and outings
  • Monitor and evaluate projects including value for money cost benefits to customers
  • Engage family, friends and professionals in all aspects of service delivery
  • Recruit and directly line-manage volunteers and befrienders, monitoring development through supervisions and training to assist them in delivering a high quality service
  • Ensure ARCO and CQC compliance.
  • Ensure the Season brand values are adhered to: overseeing the aesthetics of the internal and exterior building, staff appearance and providing a high level of customer service
  • Build positive working relationships with the catering, facilities, cleaning and maintenance contractors to ensure residents receive a seamless service
  • Actively encourage the use of the communal facilities for activities involving older people in the local community in consultation with residents
  • Promote Season and the scheme by organising open days, working with the local community to develop social integration and promoting the communal facilities within the scheme
  • Provide management with informed and objective advice and recommendations regarding issues relating to the care offer, cost, quality and delivery
  • Able to work 5 days over a 7 day week and some occasional night shifts

Person Specification

Essential

  • Track record of enthusiasm for dementia care and working with people
  • Knowledge and understanding of Dementia, and Dementia care and support
  • Knowledge and experience of CQC standards and CQC Compliance
  • Ability to drive a mini bus
  • Be a Dementia Friends Champion or willingness to attend this course
  • Be highly organised and able to work to conflicting and challenging deadlines
  • Ability to influence, motivate and lead customers and staff
  • Ability to set and deliver objectives
  • Excellent planning, communication and stakeholder management skills

Desirable

  • Recognised Social Care Qualification (such as Dip/NVQ 3 or Level 2/3 Award in Awareness of Dementia (QCF) or willingness to participate)
  • Knowledge and experience of the Enriched Opportunities Programme
  • Knowledge and experience of hospitality
  • Knowledge in older people’s finance such as benefits, personal budgets and self-payment
