Care Navigator - #2165689

We are seeking an experienced and dedicated Care Navigator to support GP practices and Primary Care Hubs across Sefton and the wider Liverpool area.

As a Care Navigator, you will play a vital role in connecting patients with GPs and other healthcare professionals, ensuring patients receive appropriate support and access to healthcare service.

Role: Care Navigator

Contract: Flexible on going hours (part-time/full-time)

Salary: £13.40 to £13.84 (PAYE inc.) / £17 to £17.50 per hour (ltd/umbrella)

Location: Services across Liverpool

Job Summary

• Contract-based opportunity within primary care.
• Previous experience as a Care Navigator or Receptionist within a GP practice, health centre, or primary care setting.
• Experience using EMIS is desirable.
• Excellent communication and interpersonal skills.
• Strong organisational and administrative abilities.
• A patient-focused approach with the ability to handle enquiries professionally.
• Ability to work effectively with healthcare professionals and administrative teams
• Provide administrative support to GP practices and Primary Care Hubs.
• Support patient enquiries and help patients navigate healthcare services.
• Previous experience as a Receptionist or Care Navigator within a GP practice or health centre is preferred.
• Provide administrative support to GP practices and Primary Care Hubs.
• Handle patient enquiries and provide information about healthcare services and procedures.
• Work closely with GPs and other healthcare professionals to support effective patient care.
• Maintain accurate patient records and databases using the EMIS clinical system.
• Carry out patient navigation duties, directing patients to appropriate services and support.
• Help ensure a smooth and efficient patient experience.
• Support the day-to-day administrative operations of primary care services.
